# Positioning for Radial Artery Harvest
- Mark sternotomy with pen
- Withdrawing the arm will take the sternum out of midline, so marking the midline will help make sure skin incision will line up with sternum. Otherwise, it will be at angles with it, producing an unsatisfying cosmetic result.
- Apply additional pulse oximeter to index finger of extremity intended for harvest.
- Put arm board out on the side of the harvest
- The angle and position of the arm board should allow the mammary retractor to be placed as necessary, and surgeon to sit cephalad or caudal to the arm. Caudal seems to work best for a right handed surgeon harvesting the left radial.
- Confirm that index finger oximeter tracing is preserved when the radial is occluded
- Rolled up towel underneath the supinated wrist to allow gentle dorsiflexion
- Another towel opened lengthwise is draped over the open hand fingers and palmar surface, then cloth tape is applied to secure the arm to the arm board.
